INCIDENT RESPONSE FOR AGENTS

Part 5 of 10

FEEDBACK LOOPS THAT PREVENT REPEAT FAILURES A feedback loop means the results of what happened are fed back to improve the system. Without it, the same incident will return. The most powerful loop for agents is turning every incident into a test. Take the exact situation that caused the failure and add it to your evaluation suite as a regression test, which is a test that checks an old bug has not come back. From then on, every release is automatically checked against that failure. Other loop outputs include updating the production readiness checklist, adding or tuning alerts, improving policies in the governance engine, adding limits to the harness, updating runbooks, and training staff. Follow up to make sure action items are completed, since a postmortem that produces a document nobody acts on has little value. Review incident patterns over time, since repeated themes, such as many incidents involving one tool, point to larger structural problems.
